Welcome to Saint Thomas More Academy! Below you will find a list of directions regarding health forms that are required by Diocesan Policy and the State of Delaware. Whether you are a new or returning student, Saint Thomas More Academy requires updated medical information each year to provide you with a safe and academically successful environment. If you are a returning student (having previously completed one or more years Saint Thomas More Academy), the following forms are due by August 15th . Please use the checklist below to ensure completion of all forms.

_____NONPRESCRIPTION MEDICATION PERMISSION SLIP
_____REGISTRATION CARD (COMPLETE WITH UPDATED EMERGENCY CONTACT INFORMATION, AS COMPLETED WITH ADMISSIONS- A COPY OF THIS CARD IS ALSO KEPT ON FILE IN THE NURSE’S OFFICE FOR EMERGENCY PURPOSES)
_____DIAA PHYSICAL (IF YOU ARE PARTICIPATING IN ANY SPORT AT STMA, PLEASE COMPLETE ALL FOUR PAGES CAREFULLY BETWEEN APRIL 1ST AND AUGUST 15TH OF THE CURRENT SCHOOL YEAR.)
If you are entering Saint Thomas More Academy as a freshman, or are a new school enterer (have not completed one or more years at Saint Thomas More Academy), the following forms are due by August 15th. Please use the checklist below to ensure completion of all forms.

_____DIAA PHYSICIAL (PLEASE COMPLETE ALL FOUR PAGES CAREFULLY, REGARDLESS OF SPORTS PARTICIPATION. IF YOU ARE PARTICIPATING IN ANY SPORT AT STMA, PLEASE COMPLETE THIS BETWEEN APRIL 1ST AND AUGUST 15TH OF THE CURRENT SCHOOL YEAR.)
The above forms must be complete and in the nurse’s office by August 15th of the current school year. All forms mentioned above (with the exception of the REGISTRATION CARD) can be found at left. Thank you for your time and cooperation.
(Note: Eden Hill Express Care in Dover, Delaware and Doc in a Box in Camden, Delaware offer DIAA sports physicals on a walk-in basis [without the requirement of insurance], for a one-time fee as of February 2012.)
