This year the administration has agreed to make all medical forms accessible through the school website. Therefore the parents can download them from home. We hope this has made things easier.
To ensure the Health and Safety of your Son/Daughter while a student at St. Thomas More, a physical form that has been completed by your physician is required. This comprehensive physical will also qualify the student for sports participation throughout the school year. The form must be completed after May 1st of the school year. You must also complete and return the Emergency Information Card. A new card must be filled out for every student at the beginning of each year.
Please find and print a copy of the DIAA Delaware Interscholastic Athletic Association) physical forms at the following link (pdf). Please note that these must include all parent signatures before your student can participate in any sports practices.
Parents- READ & SIGN: Pages 1,2 and 4
Students- READ & SIGN: Pages 2 and 4
Physicians- COMPLETE & SIGN: Pages 3, 4 and Immunization Form
All (Required) Immunizations must be completed and all dates recorded on form.
All Health forms must be completed and returned as indicated above by AUGUST 15th, 2008. NO STUDENT MAY PRACTICE FOR A SPORT WITHOUT A COMPLETED PHYSICAL ON RECORD.
Please schedule your appointment early for May 2008, since it is often difficult to get a Physical appointment during the summer months.
